Self-Assessment Medical Report
Key Points:
- Applicants must enclose a letter from their doctor which details their suitability for the JET Programme and history and treatment for any physical or mental conditions regardless of severity UNLESS they have answered 'No' to questions 3 and 4 on the Self-Assessment Medical Report.
- Applicants must disclose their full medical history as outlined in the Medical Report.
- Mild conditions will NOT preclude an applicant from participating.
- A significant discrepancy between an applicant's Self-Assessment Medical Report (submitted during application) and their Certificate of Health Form (submitted in April) may result in disqualification.
- All information is regarded as strictly confidential.
- The declaration box MUST be signed.
Part of the application form that applicants must complete consists of a section entitled 'Self-Assessment Medical Report'. Applicants are advised to complete the Self-Assessment Medical Report clearly and accurately. It is important to make sure you are fully aware of your past medical history and disclose any medical conditions you have or have had in the past.
Question 3 of the Self-Assessment Medical Report asks:
"Are you currently seeing a physician and/or undergoing treatment?"
Question 4 of the Self-Assessment Medical Report asks:
"Have you ever been treated for any mental, emotional, nervous or eating disorders, or for depression?"
If the answer to either of these questions is YES applicants MUST enclose a letter on headed paper from their GP, psychiatrist, therapist or specialist explaining their condition(s), when they started, how long the condition(s) continued or has been continuing for, the treatment involved (giving full details of any medication) and their condition now. The letter should clearly state whether the applicant is fit for life in Japan as an ALT or CIR, making sure the doctor is fully aware of the nature of the JET Programme. Applicants should also include a short note written by themselves explaining their condition(s).
It is important to note that past or current medical conditions will not prejudice your application but failure to fully disclose information may disqualify you. It is understood that this is a private matter, however providing the correct information is essential in case of any emergency situations you may find yourself in after you have departed for Japan.
Successful applicants are required to submit a Certificate of Health Form completed and signed by their GP in April. Significant discrepencies may jeopardise a participant's placement on the JET Programme.
